Prescription- Co Pay & other Medical deductions ?

Are you still able to deduct (itemize) for prescription cost's & co-pay's, doctor visit's, medical insurance co-pay ??? Also, can you can milage for having to see an out of state doctor ?? If these are eligable, is there a AGI, or limit ?? My basic's- $53k gross, single, no dep.s, & have no IRA's , and have- $4,000+ prescription cost's, $400 employer medical co-pay, + doctor visit's, etc. .. Thank You for your help ..
